Instructions for Use of the SPSE Polygraph Testing Consent Form

NOTE: This form is not meant to replace the DOE consent form, but to supplement it. When you receive notice from DOE of the date, time, and place of your polygraph exam you will also receive a medical certification form. DOE requires that the medical certification form be filled out and returned to them prior to the test. If the notice is sent to you less than 10 working days prior to the scheduled test, you may also receive a form asking you to waive your right to 10 days advance notification. Except "when good cause is shown," you are not legally required to sign the 10-day-rule waiver form and you should not do so. If you choose not to sign the waiver and also choose to assert your right to 10 days advance notification, you may call the number on the notice to reschedule the test. If you also choose to use the SPSE polygraph testing consent form, then you should follow these simple directions.
